Thank you for volunteering with BUCOP on Wednesday! Please review the following before your shift.
Clinic Details
Time: 5:00–8:00 PM
Address: 3125 E 10th St, Indianapolis, IN 46201
Parking: Gravel lot across from the clinic
Arrival:
Driving volunteers: arrive at pharmacy by 4:30 PM
Not driving volunteers: arrive at clinic by 4:45 PM
Volunteers may email you to coordinate rides.
Medications
Take photos of all invoices (front/back) and send to Kenna Teske (219-765-8744).
For insulin or brand meds (Breo, Advair, Flovent, Spiriva), complete DOH forms and send to Macy Scudder (mrscudder@butler.edu).
Fluticasone/Salmeterol may be ordered directly (1-month supply).
Incruse (umeclidinium) available for COPD; for asthma, use PAP form for Spiriva.
Updated formulary, manager roles, and DOH workflow are attached.
Before & After Clinic
Review the blue section of the tracking sheet before starting.
Leave fridge/freezer keys with cabinet keys.
Return laptops to the bottom drawer of the pharmacist counter.
Complete clinic notes before leaving:
